Job Title: Licensed Vocational Nurse – Comprehensive Care
Overview: We are seeking a compassionate and dedicated Licensed Vocational Nurse to join our team specializing in Comprehensive Care. The LVN will play a pivotal role in delivering holistic and personalized care to patients, focusing on their physical, emotional, and social well-being.
Responsibilities:

Patient Assessment and Care Coordination:
- Perform comprehensive patient assessments, considering physical, psychological, and social factors influencing health.
- Develop and implement care plans in collaboration with the healthcare team to address individual patient needs.

- Monitor and manage chronic conditions by providing education, support, and interventions to optimize patient health outcomes.
- Administer medications and treatments as prescribed, ensuring adherence to care plans.

Health Promotion and Patient Education:
- Educate patients and families on preventive health measures, lifestyle modifications, and disease management strategies.
- Facilitate health education programs and workshops aimed at promoting overall wellness.

Care Coordination and Advocacy:
- Coordinate healthcare services and referrals, ensuring seamless transitions between various healthcare providers and facilities.
- Advocate for patients’ needs, collaborating with community resources and support services to enhance patient care.

Documentation and Reporting:
- Maintain accurate and comprehensive patient records, documenting assessments, interventions, and outcomes.
- Generate reports and summaries outlining patient progress and participate in care conferences as needed.
Qualifications:
- Active LVN license in Texas.
- Previous experience in comprehensive care, primary care, or a related healthcare setting is preferred.
- Strong clinical assessment skills and the ability to address diverse patient needs.
- Excellent communication and patient education abilities.
- Ability to work collaboratively within a multidisciplinary team.
Preferred Qualifications:
- Familiarity with electronic health records (EHR) systems.
- Additional certifications or coursework in comprehensive care, community health, or patient advocacy.
