**Telemetry Technician – ICU Telemetry @ Swedish Issaquah**

**Full Time**

**12 Hour Shift**

The monitor tech I (MT) is responsible for continuously monitoring patients via telemetry of oxygen saturation (Sp02) levels and/or electrocardiogram (ECG) rhythm on patients from a remote workstation located in the TeleICU Clinical Operations Room (COR). The Monitor Tech communicates all concerns to staff registered nurse (RN). Utilizes TeleICU RN (tRN) as a resource. If the MT is unable to reach the bedside RN in a timely manner, the MT may escalate the concern by calling the charge nurse, Rapid Response Team or Code Blue per unit procedure. Additionally, the MT provides clerical support to the workflows of the TeleICU clinical staff (tMD and tRN), assists and participates with the orientation of new MT and nursing staff, and other duties as assigned.

General Customer Service:

Greets approved visitors/tours, providing information and assistance as needed. Works with physicians, nurses and other hospital staff to identify needs, resolve problems or provide other information. May facilitate patient movement to other departments. Answers phone calls, providing staff assistance as appropriate. May answer eLerts and triage to RN/MD.

New Admissions, Transfer & Discharge Activities in eCareManager:

Ensures current ADT and patient data is importing, interfacing and trending to and from eCareManager, EMR, Bedside Monitors, etc. Ensures continuous trending for all ICU and IMCU assigned monitored beds in the system at all times by utilizing eCareManager notifications, text pager notifications, and other communications from staff. Updates patient census: eCareManager patient lists, EMR, assignment sheet/logs, and monitoring screens.

Electronic Medical Record “EMR”:

Performs all ADT (admission, discharge and transfer) functions. Runs reports and audits as requested by unit manager or designee. Facilitates entry of non-medication verbal orders during a “Code Blue” or other emergencies. Uses computer, telephone or requisition to enter physician orders, diagnostic tests or other procedures. May act as a resource for the EMR for staff and physicians.

Environment of Care & Regulatory Readiness:

Rounds the unit in accordance with department regulatory standards i.e. keeps hallways clear, workstation and equipment plugged in and functioning at all times. Has knowledge of emergency backup equipment and downtime procedures. Ensures downtime computer is on and checklists are complete. Possess ability to troubleshoot equipment and software issues. Communicates with other hospital departments regarding concerns with faulty equipment, work orders or other issues, and escalates safety concerns appropriately. Utilizes eQVR for reporting all safety issues and “great catches”. Keep work area hygienic, clean and clutter free. Participates in the unit’s monthly cleaning responsibilities. Actively participates in unit, department, and system-wide educational activities including Quality Improvement programs as appropriate.

Other Duties:

Gives report to relief staff, including items that require follow up. Updates patient census and assignment logs and various monitoring systems (Philips,Spacelabs, eCareManager, Masimo, paper forms, etc.). May assign pagers and locators to nursing staff. Maintains unit worksheets and records. Distributes mail. Sends patient reports to HIM upon discharge when necessary. Maintains confidential shredding practices. Inventories education brochures and office supplies, and orders as appropriate. Notifies nurse manager and/or designee and makes recommendation for par level adjustments. Judiciously orders supplies from MSC, Nutrition etc., as necessary. Facilitates staffing coverage with charge nurses and Nursing Resource Office with fiscal awareness. Performs other duties as assigned, to insure effectiveness of care delivery and unit function.

Continuous Remote Monitoring:

* Continuously monitors patients via remote Sp02 monitoring; responds to the Sp02, RRA, HR alarms and trending. Communicates significant respiratory changes by alerting the bedside nurse immediately.

* Ensures and communicates with the bedside staff that patient identifiers are used to assign Sp02 device upon initiation/discontinuation of patient

monitoring. Admit, discharge and document assignment information on patient log and Masimo monitoring system: patient name, room number,

monitor and pager assignment, and RN name on TeleICU worksheet.

* Continuously views and interprets patients via remote cardiac rhythm monitoring. Monitors patients in leads V1 and III unless there is a clinical reason to use different leads. Communicates changes in heart rate, rhythm, and other significant patient information immediately to bedside RN.

* Records each patient’s heart rate, rhythm, and ectopy each hour on monitor log. Admits, discharges, transfers patient within the monitoring system and confirms ADT with bedside RN. Documents each patient’s baseline rhythm strip and any significant changes in rhythm in monitor log and posts a note in EMR as outlined in Monitor Technician Guidelines. Posts HR, rhythm, and ectopy every 4 hours and/or with each set of bedside vital signs in EMR.

* Notifies bedside RN immediately when batteries are low in telemetry or oximetry units or when signal is lost due to artifact or patient readings out-of-range.

* Escalates unresolved patient safety monitoring issues to the appropriate stakeholders: bedside RN, TeleICU RN consultation, charge nurse, hospital supervisor, manager, etc. The monitor tech may activate RRT or Code Blue in collaboration with TeleICU RN.

* Validates patient assignment information is correct during hand-offs such as the beginning of each shift, coverage for breaks and at the nursing unit change of shift.

TELEICU / eICU:

* Enters I/O, height, weight, temperature and other data as assigned, delegated by nursing in eCareManager. Ensures patient vital sign flowsheet,

intake/output, and bedside trending information is importing/interfacing to the COR.

* Completes, processes and files assigned quality improvement rounds/audit tools to the appropriate departments. Utilizes eCareManager and EMR as

needed to complete rounds and audits.

* Answers and triages phone calls to the eMD and/or eRN to support the TeleICU triage model. May assist in answering eLert calls from the ICUs when

clinicians are actively consulting.

* Data processing for TeleICU productivity metrics.

Answers alarms and warnings and elevates patient needs to nursing staff.

**Required qualifications:**

+ Successful completion of Basic Cardiac Arrhythmia Interpretation class with a score of at least 80% on final exam.

+ Individuals hired with previous monitor technician experience must pass a challenge exam with a score of at least 80%.

+ Required upon hire: National Provider BLS – American Heart Association.

+ 1 year Recent ECG monitoring experience managing patients on cardiac monitors in a telemetry, neurological, cardiac ICU, ICU or TeleICU.

**Preferred qualification:**

+ upon hire: Monitor Tech Certification

+ To be promoted to a Monitor Tech II level, EICU Monitor Tech must successfully complete 3 months of orientation with one on one training in the ICU with an assigned Preceptor. After the 3 month orientation period, will then complete 9 months of experience in the EICU monitoring patients with an assigned resource support staff member. At the conclusion of the 9 months of training experience must be able to independently monitor a patient load of 35 patients and successfully pass competencies. At that time Employee may qualified to promote to a Monitor Technician II level.

+ Previous experience using electronic medical record.
