**Job Summary**

Analyzes and completes assigned chart corrections including potential chart duplications, limited contact moves and basic chart correction processes to uphold the integrity of the medical record and master patient index. Researches patient’s identity and documentation for purposes of merging and to ensure correct demographics are being used. Follows up and completes assigned tasks related to chart correction in timely manner. Works with close supervision and within established policies and guidelines. Analyzes assigned tasks and utilizes specific tools following documented protocols and polices related to chart correction. Will communicate tasks and directions to clinical and non-clinical users, providing clear, concise instructions and timely responses. Must be able to review patient demographics and other PHI contained in various sources to determine correct patient identity. Maintains required information for tracking purposes and updates tasks as directed. Completes tasks in expected turnaround times. Must have a basic knowledge of HIM and provider documentation workflows. Is able to prioritize workload. Must understand healthcare terminology and basic processes associated with hospital/clinic practice. Must have a basic knowledge of medical record content along with a good working knowledge of the EHR either through experience or education.

**Qualifications**

High school diploma required. Health information technology or similar field preferred.

Minimum of two years’ experience in a multi-facility health information management (HIM) or similar healthcare setting. Previous experience with Sanford One Chart or Epic preferred.

Registered Health Information Administrator (RHIA) and/or Registered Health Information Technician (RHIT) preferred
