*Position Summary:*
This position supports the program implementation through patient outreach and collaboration, data collection program evaluation, patient education, and coordinating patient care. This requires contact with patients, outside agencies, and the emergency department as well as outpatient staff to provide referrals for resource and management as patients learn to navigate the health care system.
*Minimum Qualifications:*
* Must have a high school diploma or equivalent
* BLS is required within 60 days of hire
* Basic knowledge of substance use disorders.
* Demonstrated human relation and effective communication skills required.
* Excellent organizational skills and ability to multitask. Experience with community and patient engagement.
* Must be familiar with HIPAA. Applicants must demonstrate the potential ability to perform the essential functions of the job as outlined in the position description.
