*Job Summary*

This positionsupports program implementation through patient outreach and collaboration, data collection, program evaluation, patient education, and coordinating patient care. This requires contact with patients, outside agencies, and the emergency department as well as outpatient staff to provide referrals for resources and management as patients learn to navigate the health care system.

*Essential Functions*

1. Builds relationships with patients, health care teams, and resource providers.

2. Coordinates with patients and community providers to ensure appropriate follow-ups are being completed. Creates and maintains up to date resource lists and community contacts.

3. Engages directly with hospital and ED staff as a liaison and representative of the Bridge program.

4. Explains and obtains informed consent, medical history and demographics; documents and maintains with historical data, progress notes, and patient log to help ensure safety.

5. Performs patient screenings for opioid use disorder, opioid withdrawal, and comorbid conditions.

6. Initiates strategies to promote enrollment and patient compliance.

7. Identifies patient needs and directs to/coordinates appropriate resources.

8. Excellent organization and communication skills in coordinating outpatient follow up after emergency departments visits.

9. Knowledge of opioid use disorder, naloxone administration, and ability to educate patients on naloxone use.

10. Knowledge of or ability to learn and stay updated on community and statewide resources as they pertain to opioid use disorder.

11. Contributes to developing educational materials and educates the community and other professionals regarding the program.

Demonstrated human relation and effective communication skills required.

Basic knowledge of substance use.

Excellent organizational skills and ability to multitask.

Experience with community and patient engagement.

This position is patient-sensitive and must fulfill all associated requirements.
