*_Position Purpose:_*
The Care Coordinator is responsible for the management of the StepOne Service office, which includes managing telephone inquiries concerning the StepOne Service, screening potential adult patients, assisting with patient questions and concerns, and initiating discharge planning.
The Care Coordinator coordinates the service within the hospital andfunctions as a resource to the nursing staff, unit supervisors and the EHP leadership team to coordinate a continuum of care.
*_Essential Functions for Screening and Intake _*
Effectively screens and reports patient information
· Able to engage patients during telephone inquiries utilizing effective communication and listening skills.
· Able to recognize and understand the signs and symptoms of substance use withdrawal to determine appropriateness for admission.
· Able to understand and apply knowledge of medical necessity criteria for admission.
· Able to articulate patient information to the physician in a clear, concise manner.
· Understands and completes all intake forms including the assessment tools required to confirm medical necessity for admission.
· Initiates discharge plan during the initial face-to-face encounter.
· Assist the Service Coordinator in community outreach by telephone.
· Chart audits and other reports, as requested.
· Assist the Service Coordinator with other duties as assigned, including, but not limited to: tours of unit; distributing informational materials within the hospital.
· Enter program data into required spreadsheets.
· Follow-up on inquiries meeting criteria but not yet admitted.
· Ensures that work area is clean, secure, and well maintained.
· Participates in staff meetings and attends other meetings and seminars as directed.
· Stays well informed of EHP Corporate policies and procedures.
· Participate in the training of new employees when required.

· Maintaining and expanding knowledge and understanding of the field of substance use disorders.
· Seek experiences and continuing education/training that enables the Care Coordinator to work more effectively in the field substance use disorders.
· Accepts accountability for information contained in the EHP Policy and Procedure manual.
*_Communication Skills_*
Utilizes necessary verbal and written skills required to support the efficiency of the service*__*within the client hospital.
· Demonstrate effective verbal and listening skills when interacting with supervisors, peers, and client hospital staff, patients, families and community agency representatives.
· Demonstrate clarity, accuracy and conciseness in preparing all forms of written communication.
· Demonstrate promptness, courtesy and effective communication skills when conducting telephone contacts with the community.
· Maintain patient confidentiality and privacy.
