Overview
**Description**
The Community Health Worker (CHW) will assist the Project Manager to meet the objectives of the Well-Being Trust Grant by promoting mental health, substance abuse recovery and resilience to identified community members. The CHW will assist the identified patient in accessing community services and will educate the patient and family regarding the plan of care. The CHW will act as an advocate to decrease disparities in social determinants of health among vulnerable populations such as cultural factors, housing language obstacles and knowledge of how to access social services and other resources. The services provided by the CHW will occur primarily in the patient’s place of residence. The CHW will exercise some independent judgment in developing, planning, coordinating, and delivering services with guidance from the Project Director.

**Required qualifications:**
+ Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
**Preferred qualifications:**
+ Coursework/Training: Medical assistant certification
+ Work experience in community setting.
