**Description**
**Diabetes Educator- this position travels to our Providence Medical Groups/Clinics – Portland Metro Area (Part-Time/Day Schedule)**
The diabetes educator provides education for clients with diabetes of all ages according to the individualized needs of the client. This process involves a health and education assessment, development, implementation and evaluation of an education plan, taking into account psychosocial, economic, and language barriers. Duties are performed in a classroom or individual outpatient setting in collaboration with a team of RNs & RDs. The candidate accepts professional development and community outreach as an integral part of professional practice.

**Required Qualifications:**
+ upon hire: Oregon Dietitian License.
+ upon hire: National CDR Registered Dietitian – Commission on Dietetic Registration.
+ 3 years Experience in treatment and education of diabetes within the last 2 years.
+ Experience with current adult learning theory in relation to diabetes education.
+ Experience with the clinical management of diabetes.
**Preferred Qualifications:**
+ upon hire: Certification in Diabetes Education, or willingness to attain CDE certification.
+ Coursework/Training Formal coursework or credentials in principles of adult learning theory and patient-centered learning.
+ Experience with diverse ethnic groups.
