*What you’ll do:*

As a Director, Clinical Post Acute Services, you will lead the Long Term Care clinical metrics and support across Lifepoint Hospitals. The position functions to provide intensive resource management related to clinical appropriateness, operational efficiencies, financial optimization, professional development, and leadership mentoring. The Director is also responsible for compliance with federal, state and local regulatory standards and guidelines. This position also assists in the training, development and mentoring of clinical and operational long term care senior leaders and hospital case managers. The Senior Director will be responsible for interfacing with facility clinical gatekeepers and others involved in quality, resource management, and discharge planning. Additionally, this position will lead these services in coordinating the company’s accreditation activities related to long term care services, including performing on-site reviews and consultations. This position will be focused on Long Term Care including nursing home and assisted living and swing bed optimization with the intent to grow responsibility to additional post acute and population health responsibilities as these strategies are developed across the organization.

*Responsibilities:*

· Works within the Post acute services framework by optimizing Long Term Care and Assisted Living facilities

· Ensure that Long Term Care leadership teams participate in company-wide clinical professional development and clinical leader mentorship/development activities;

· Works in concert with the Post Acute Resource Team members to create, foster and maintain a culture of safety and quality throughout all post acute organizations;

· Effectively communicate with group leadership team members and facility leadership/staff teams appropriately and within the Post Acute Structure regarding updates, identified issues or process improvement activities within the services.

· Provide guidance and leadership to Post Acute team and facility leadership/staff on regulatory and compliance issues as identified or as industry changes occur.

· Possess excellent communication skills in mentoring leadership and staff members within the Post Acute Service locations.

· Ensure, obtain and compile a monthly operational and quality report that consolidates all long term care findings and promotes early identification of negative trends and or best practices

· Complete monthly or as needed conference calls with facilities to promote and develop a strong knowledgeable team

· Become the go-to person for long term care & assisted living clinical care across the organization.

· Orient and develop strong leadership teams within the long term care service locations in order for successful succession plans to be implemented as necessary.

· Review outcome information (MDS) and identify reimbursement, financial and clinical operating indicators on an ongoing basis for potential issues or trends and implement interventions or corrective action plans as indicated;

· Collects, compiles, analyzes and interpretsperformance improvement data and presents to the Post acute and other key leadership teams on a quarterly basis

· Ensure that PAC leaders and managers remain current on accreditation standards and regulatory requirements;

· Conduct pre-survey assessments for readiness approximately 4 months prior to each facility’s anticipated survey;

· Prepare and present educational programs

· Act as a liaison with other departments and outside resources to insure delivery of safe, effective quality care.

· Provide operational evaluation, development, and implementation and variance determination though on site and data metrics review monthly.

*What you’ll need:*

· Undergraduate degree required: advanced degree in Business Administration, Nursing (or other related field) desirable.

· A minimum of five (5) years of increasing responsibility in managing long term care facilities and at least eight (8) of years in the healthcare industry (medical/surgical, hospice, geriatrics nursing experience).

· In depth knowledge of PAC operations, regulations, financials, and CQI tools.

· Formal training or certification in ISO, Six Sigma, LEAN, CPHQ a plus.
