**Description**

The LD Case Manager works with nurses, social workers, physicians, other practitioners, caregivers and the community. The Care Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of Care Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient’s right to self determination.

Under the direct supervision of the Director of Case Management or designee, and in collaboration with the nursing, medical, and ancillary staff, the Lead Case Manager is responsible for care management to achieve cost-effective utilization of resources through the proactive coordination of services in order to move patients efficiently through the continuum of care. The Lead Case Manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors the effectiveness of interventions to meet patients’ treatment, transition of care needs and department goals. **Required Qualifications:**

+ Bachelor’s Degree Nursing.

+ Upon hire: California Registered Nurse License

+ 3 years Minimum of three years of acute nursing experience within the past 5 years.

**Preferred Qualifications:**

+ Master’s Degree master’s degree in nursing strongly preferred.

+ Upon hire: National Certification

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
