**Providence is calling a Navigator RN who will:**

+ Be responsible for providing direct patient care navigation improving patient experience through promoting and ensuring timely, seamless and appropriate care

**We welcome 100% remote work for residents who reside in one of the following States:**

**Essential Functions:**

+ Triages patients using recent lab results to the appropriate level of care

+ Works closely with clinicians and advanced practice providers to assist with referral management

+ Uses evidence-based resources to provide guidance for follow-up care following screenings

+ Ensures timely interventions by providing consultation and follow-up with clinicians and APPs

+ Communicates recommended screenings and care plan instructions to patients

+ Uses EMR reporting tools to identify patients needing outreach

**Job Specific Knowledge, Skills & Abilities:**

+ Medical terminology

+ Familiarity with Managed Care (HMO, PPO, POS, PSO, and Capitation)

+ Effective communication skills

+ Critical thinking skills

+ Maintain professional relationships, work effectively and collaboratively with other team members

+ Maintain knowledge regarding standards of care

+ Ability to concentrate on tasks, juggle multiple competing priorities, handle stress under tight time pressures, problem solve, and follow through on execution of care plans and follow-up care

**Required qualifications for this position include:**

+ Nursing Degree from an accredited school

+ 3+ years professional nursing experience

+ Unencumbered RN Licensure in state of residency: California, Washington, Oregon

**Preferred qualifications for this position include:**

+ Bachelor’s Degree Nursing

+ Bilingual: Fluent Spanish and English **-OR-** Bilingual: Fluent Tagalog and English

+ Max: $79.69

+ Min: $46.00

+ Max: $74.28

+ Min: $43.91

+ Max: $70.91

**NorCal (Napa, Sonoma):**

+ Min: $57.71

+ Max: $93.19

**SoCal, NorCal (Humboldt):**

+ Min: $51.44

+ Max: $83.06
