JOB SUMMARY
The oncology patient navigator functions on a multidisciplinary team providing coordination services for those patients receiving oncology care. The patient navigator serves as a clinical resource for education acting as an advocate to assure comprehensive services throughout the continuum of care. The patient navigator serves as a liaison between MPH and the community services and support.
Reports to: Director Oncology
ESSENTIAL FUNCTIONS
Navigation of breast and GI patients, head and neck patients, including, arranging appointments with oncology physicians in an appropriate time manner, as needed for the patient.
Assures that communication between patients’ physicians is made and advocate for the patient, to assure needs are met.
Provides support throughout treatment, in providing resources as needed, whether it be financial, mental support, assisting with pre-surgical and post-surgical needs, such as arranging fittings for post-surgical prosthetics, etc.
Along with the duties of patient navigation, individual is involved in the COC accreditation for our oncology department and help with our support group.
Helps with the breast screening and prostate events that are hosted annually.
Attends Tumor Board and communicates to physicians within these meetings the patients that are being navigated as they are discussed.
Helps with prior authorizations, as needed for patients, and also with patient survivorship plans.
Is a companion to physicians as needed with patient exams, and provides help to physicians as needed, with referrals to other physicians in a timely manner.
Sends referrals and coordinates genetic testing for patients in the oncology department, and follows patients through with this appointment, in retrieving saliva samples, from patients to be tested.

JOB REQUIREMENTS
Minimum EducationHigh School Diploma or GED required.
Minimum Work ExperienceHealthcare experience preferred.
Required SkillsLicenses:American Heart Association (AHA) BLS required.Certifications:Navigation certification preferred.
