*Position Summary:*

Provides comprehensive pharmaceutical care to CCH patients, including drug information, therapeutic drug monitoring and pharmacokinetic dosing, renal dose adjustment, therapeutic interchange, IV to PO conversion, and patient education / counseling. Serves as a subject matter expert for the healthcare team regarding drug therapy. Advocates safe medication use. Aligns prescribing, dispensing, administration, and monitoring functions with established policies and procedures for safe practice.

1. Enters medications orders accurately, including therapeutic screening for safety, efficacy, and formulary compliance.
2. Assures correctness and regulatory compliance for preparation, labeling, and delivery of medications.
3. Provides Pharmacy-to-Dose services, including aminoglycosides, vancomycin, and anticoagulation.
4. Demonstrates understanding and effective use of Pharmacy policies, including therapeutic interchange, renal dose adjustment, IV to PO de-escalation, etc.
5. Educates hospital staff regarding drug information, safe medication practices, drug use policy and procedure compliance, etc. as needed.
6. Suggests drug therapies and monitors patient responses and reactions to drugs.
7. Assists in maintaining and complying with the Formulary.
8. Monitors and helps resolve Pyxis discrepancies.
9. Instructs patients on medication safety involving Warfarin or other agents as requested.
10. Documents and submits medication occurrences and adverse drug reactions appropriately.
11. Communicates with prescribers regarding drug shortages and alternative therapies as needed.

*Required Education*:

Bachelor’s degree is required

Doctoral degree is preferred

Experience in hospital or similar Pharmacy setting preferred. Strong computer skills essential. Ability to communicate clearly in written and oral forms required.
