*Position Summary:*
Provides comprehensive pharmaceutical care to CCH patients, including drug information, therapeutic drug monitoring and pharmacokinetic dosing, renal dose adjustment, therapeutic interchange, IV to PO conversion, and patient education / counseling. Serves as a subject matter expert for the healthcare team regarding drug therapy. Advocates safe medication use. Aligns prescribing, dispensing, administration, and monitoring functions with established policies and procedures for safe practice.
1. Leads, guides, and represents Pharmacy through departmental and/or system-level information systems changes such as software conversions, electronic health record migrations, etc.
2. Leads and facilitates implementation and ongoing optimization of clinical information systems, automation tools, and corresponding workflow for the Pharmacy Department.
3. Facilitates communications between IST, other technical support personnel, and end users to ensure continuity of service during downtime events, change management requests, etc.
4. Collaborates with IST to assess, communicate, and fulfill hardware and software needs.
5. Represents Pharmacy on the IT Steering Committee and conducts Pharmacy/IT compliance activities such as ongoing security/access reviews, HIPAA compliance audits, etc.
6. Engages end users to resolve operational issues involving clinical informatics.
7. Effectively communicates changes or updates in functionalities to end users.
8. Queries Pharmacy databases, performs data analytics, and supports operational leadership by turning medication use data into decision support information.
9. Assists Pharmacy Director in reporting compliance, quality, and performance measures.
10. Assesses Pharmacy staff capabilities in the use of clinical information systems, supporting ongoing training, education, etc. as needed.
11. Develops and maintains collaborative working relationships with physicians, clinicians, hospital administrators, and hospital departments to align Pharmacy clinical informatics activities with patient-centered best practices.
12. Assists in maintaining the Pharmacy Formulary, infusion pump library, Sentri-7 rules and reports, and other dynamic information systems functions that support clinical Pharmacy services and/or medication use.
*Required Education*:
Bachelor’s degree is required
Doctoral degree is preferred
Experience in hospital or similar Pharmacy setting preferred. Strong computer skills essential. Ability to communicate clearly in written and oral forms required.
