*Registered Nurse (RN) – Case Manager*
*Days (Monday-Friday, 8a-4:30p, occasional weekends, no call)*
*$10,000 Sign-on Bonus Eligible*
*Position Summary: *
* Monitors and manages patient care to promote continuity of care,optimalpatient outcomes, patient satisfaction, costefficiency, and compliance. Mustmaintainup to date knowledge of regulatory agency changes related to case management.
* Serves as a care coordinator tomonitorand coordinate a comprehensive program of admissions reviews, continued stays and discharge plans of the patient.
* Processesappropriate datatomonitorand guide acute care hospital coverage for patients. Conducts medical record audits and evaluations.
* Prepares statistical analysisinaccordance withdepartmental, organizational, and regulatory agencies requirements.
* Initiates and directs caremanagement of patients from preadmission through discharge forappropriate movementthrough the continuum ofcare whileprovidingquality care advice to meet specific patient needs.
* Monitors cases,developsand implementsplans forappropriate documentationand length of stay compliance.

*Minimum Qualifications: *
* Graduate of accredited School of Nursing. Valid Viriginia (or compact) RN licenserequired.
* Current AHA BLS isrequired.
* Certification as Case Manager (preferred).
* At least one year of clinical direct patient care nursing experience in an acute care hospitalrequired.PreviousNursing/Case Management/Utilization Review/Discharge Planning experience preferred.
* Requires critical thinking skills, decisivejudgmentand the ability to work with minimal supervision. Must be able to work in a stressful environment and takeappropriate action.
* Ability to communicate effectively.
* Knowledge of Basic Computer Skills.
* Ability to work under high stress situations andmaintainprofessionalattitude.
* Ability to accept responsibility. Ability to follow policies and procedures.
* Demonstrates discretion in handling materials of confidential nature.
